Transition Tool Kit Appendix

Autism Speaks Transition Tool Kit was created to serve as a guide that can provide you with options to think about and explore as you and your adolescent with autism begin to plan for adulthood.

We have created a Transition Tool Kit Online Appendix where families can find additional tools, resources and tips that may be helpful during the transition process. The appendix will be a live database where items will be added and updated regularly. We know how valuable it is for families and professionals to learn from one another. So if you know of any items that you think may be helpful to other families, please submit them to us at and we will include them in the Online Appendix.
We have broken the appendix into groups that correspond to specific topics of the Tool Kit.
